Hospice

The Home Health Care Team includes:

• Registered nurses who provide skilled nursing care developed for each patient
• Certified Nursing assistants who help with personal care and other activities
• Physical, Occupational and Speech Therapists who work to increase patients’ mobility, safety, independence and communication skills
• Medical Social Workers who identify resources for patients, assist with problem solving and provide short term counseling

Hospice is a concept of caring that focuses on comfort, rather than curative treatment. We offer a comprehensive and coordinated program of care honoring the unique physical, emotional and spiritual needs of each child, adult and family member based on the patient’s wishes and family’s needs. Pain and symptom management is paramount in our care.

Hospice affirms life and regards dying as a normal process. The goal is to assist patients and families to attain the necessary preparation for a death that is compassionate, peaceful and dignified.

Confluence Health Hospice provides this care in the patient’s home, a nursing home or wherever the patient resides. Our services are available 7 days a week, with on-call triage after hours, weekends & holidays. Learn more about hospice services.
